Community Health Needs Assessment (CHNA)
A Community Health Needs Assessment (CHNA) is a process by which we evaluate the health needs of the community we serve. The assessment is conducted every three years, and helps us identify the needs, gaps in services, assets, and resources as they relate to the health of the community. With the results of the assessment we can plan, manage, and evaluate community health initiatives and services to address the needs and priorities identified by the assessment. This work is done to help address disparities in health and health care delivery and to improve the overall health and wellbeing of our community.
